| In applying to Logan Christian School, there are several forms that must be filled out and signed by prospective families. An application will be accepted only when all items are completed and received: 1. Application with a recent photo of the child 2. Current Immunization record 3. A copy of the birth certificate 4. A copy of the child's social security card 5. Registration fee 6. Previous year's report card, for students grades 1-6 7. Custody documents, if any |
